Better Choices Better Health Jan 2009-present

Maui AgeWave, LLC has coordinated the delivery of the Better Choices Better Health Program since January 2008.  Duties include coordinating logistics for  classes, attracting participants, maximizing the number of trained facilitators, coordinating volunteers and assuring the evidence based deliverables are prepared to facilitate program funding. 

Better Choices Better Health is a Stanford University Program and is brought to the Maui Community through Maui County Office on Aging.  The program is a special project for the State of Hawaii Healthy Aging Partnership and is supported by a wide array of community partners.

Maui County Office On Aging Four Year Area Plan April 2011 to October 2011

MAW was retained as the Project Coordinator to facilitate the development of the Maui County Office on Aging’s  4-year Area Plan on Aging, covering the federal fiscal period from October 1, 2011 through September 30, 2015.  It was developed to be a practical and comprehensive planning and compliance document to guide the County of Maui to assure appropriate, high quality services are delivered to the tri-island county’s older adults.  The creation and adoption of the plan is a factor in the ability of the County of Maui to receive federal funds under the Older Americans Act through the federal Administration on Aging and the state Office on Aging. 

This plan describes the Older American Maui County population and the approaches to be taken to provide them an appropriate and coordinated system of services that fully supports their well-being. The carefully developed array of services provided offers our seniors a backdrop for assuring that Maui County Seniors can age with the grace and dignity encompassed in the Hawaiian value known as ‘aloha’.

Community Living Project - CLP September 2011 - June 2013

The State of Hawaii has selected Maui AgeWave (MAW) to deliver a pilot CLP Program to the areas of Hana, Lanai and Molokai – the most rural and underserved areas in the county of Maui.

The Hawaii Department of Health, Executive Office on Aging (EOA) will provide this pilot program of participant-directed supports (called Hawaii’s Community Living Program or CLP) to adults with limited incomes and assets, but not Medicaid eligible, who may be at risk of placement in a nursing facility due to functional challenges. The purpose of the program is to help these eligible adults remain living in their own homes and avoid impoverishment. The program serves participants by providing a “Coach” and a monthly budget to purchase supports such as employing personal assistance in the home or goods that support the participant to remain living at home. Federal grant and state funds are allocated to provide direct supports (funding the individual monthly budgets) for participants in Maui County during a 12-month period.

Financial Management for Seniors 10-1-11 to 6-30-12

MAW has been selected by Maui County to provide seniors with help and assistance in organizing and managing their finances.  The service is available to seniors 60 years and older who are clients of Maui County Office on Aging.  Services include:
  • Obtain assistance with finances

  • Preserve assets

  • Reduce debt burden

  • Discuss financial options and concerns

  • Manage financial resources more effectively

  • Bill paying in an orderly way

  • Review of financial commitments

  • Budgeting and tracking

  • Build skills and confidence around financial performance

Financial Management

 

Department of Health Telehealth Pilot Project January 2008-June 2009

MAW was a partner and champion in an innovative, forward-thinking pilot project in partnership with the Maui District Office of the Department of Health.   This pilot focused on using two-way audio visual connections via laptop allowing homebound individuals to communicate with public health nurses who were located in their offices during the visit.

A total of 60 in-home virtual visits were done with the intent of determining if tele-visits could eventually replace some of the in-person visits by Public Health Nurses with the hope of reducing system costs and improving care outcomes. This experience was invaluable in observing the individual learning styles of senior citizens and noting how they compensated for their weaknesses with alternative processes.
